Statement of Purpose

About us

Beyond Vision Transitions (BVT) has been established by experienced professionals with qualifications and expertise in social care, rehabilitation, quality assurance, and education. We are a high-quality supported living service for vulnerable adults and children who require safe accommodation and personalised support.

We provide a high-quality supported living service for vulnerable adults and children, offering a range of care options including 24/7 supported accommodation, floating support, and outreach services.

Our support is designed for:

  • Young people in care or leaving care

  • Adults and young people facing homelessness

  • Individuals with eating disorders, autism, learning disabilities, or mental health needs

  • People with complex or challenging needs

We work closely with local authorities to create tailored support packages that meet the unique needs of each individual. Our aim is to provide a safe, secure, and nurturing environment where both adults and young people can:

  • Feel valued and understood

  • Receive consistent, professional care

  • Develop life skills and resilience

  • Overcome challenges and work towards independence

 

For young people, we carry out thorough assessments to ensure we can meet their emotional, behavioural, and educational needs. Our care is person centred and therapeutic, with consistent boundaries that promote safety and stability.

For adults, we provide support that empowers them to maintain independence, manage their wellbeing, and access community resources. We focus on enabling personal growth, building confidence, and promoting healthy, positive lifestyles.

Our dedicated staff are genuine role models, trained to provide guidance and support through professional, trusting relationships. Using an integrated care approach, we collaborate across care, therapy, and education to ensure every person’s voice is heard and their best interests are always at the heart of care planning.

WHAT WE PROVIDE

At Beyond Vision Transitions we provide the following services:

A SUPPORTED HOME

Beyond Vision Transitions accepts both emergency and planned placements. Upon referral, we carry out a comprehensive assessment to ensure we can meet each individual’s emotional, behavioural, social, and practical support needs. Our care is person-centred, focusing on enabling people to live safely, confidently, and as independently as possible within the wider community. We provide a safe, secure, and supportive environment with consistent boundaries that promote stability and wellbeing.

We place great emphasis on personal development, including education, vocational skills, and community engagement. For young people, we actively encourage and support educational achievement. For adults, we focus on developing life skills, employability, and opportunities for meaningful participation in society.

Our aim is to create an environment that supports healing from past trauma while promoting empowerment, independence, and informed choice. We work to enhance daily living skills, build self-esteem, and prepare individuals—whether transitioning to adulthood or towards greater independence—for a fulfilling and sustainable future.

MODERN LIVING FACILTIES

We ensure that our floating support homes are fully equipped to meet the requirements of all people, including on-site laundry facilities. All living areas, communal areas and bedrooms will be inspected on a regular basis to ensure home rules are being complied with. We provide staffing for all needs including basic activities i.e. cooking and cleaning and support on a one to one basis. We ensure all accommodation needs are met in regards to health and safety standards and recommended environmental health standards, including the installation of mains smoke alarms, fire regulated furnishings and safety locks.

1-1 KEY WORK SUPPORT

The aim of our service is to provide a safe and homely environment that promotes empowerment, independence and choice, whilst enhancing individual’s daily living skills and enabling opportunities to move on to an unsupported setting where possible. Our goal is by the time people leave us, they’ll be able to become a positive contributor to society and we would have aided in their transition to independence and adulthood.

MENTORING & LIFE SKILLS

When every day activities and thoughts become too difficult to manage by ourselves, we often need our support network to chip in. Beyond Vision Transitions provides the positive reinforcement, life coaching, mentoring schemes and proactive engagement required to re-direct young people into their future paths.

OUTREACH SUPPORT

Our Outreach Support Service provides tailored assistance for adults aged 18+ who have moved on from supported living but still need a helping hand to maintain independence in their own home. We offer flexible, person-centred support with daily living skills, budgeting, accessing community services, managing tenancy responsibilities, and emotional wellbeing. Our goal is to ensure individuals feel confident, connected, and fully supported as they continue building a stable and fulfilling life.

ASSISTANCE & ADVICE RE: EDUCATION, WORK AND TRAINING.

Our skilled and compassionate staff provide practical assistance and advice on education, employment, and training. We work closely with each individual to identify their goals, explore opportunities, and develop the skills and confidence needed to succeed. From supporting college applications and vocational courses to helping with job searches, interview preparation, and workplace skills, we empower adults to take positive steps towards independence and a brighter future.
